That is an excellent question—and one many people never think to ask.

You know what the bank concluded because it sent you a denial. But do you know what records it reviewed? What evidence it considered? Whether anyone compared the transactions to your normal account activity? Whether anyone examined what happened before the money disappeared?

Sometimes all you really receive is the result: “We investigated. No fraud found.”

The word “investigation” should not prevent you from asking what the investigation actually consisted of.
